Guidelines for Obtaining a Birth or Death Certificate
For Applicants
Vital Records Division
The information are the guidelines for an applicant obtaining a birth or death certificate. Originals of Support Documentation is Required.
- A non-expired government-issued photo identification issued by an appropriate issuing authority alone is typically sufficient to obtain a copy of one’s own birth certificate
- Other documentation that establishes a preponderance of evidence of identity may be required
The Registrar may at any time request additional documentation to support the determination of identity or eligibility of the applicant
At least three (3) pieces of documentation are required to establish a preponderance of evidence of identity, Examples include:
- Employment Identification card with photo, accompanied with pay stub or W-2
- School, university, or college identification card with photo, accompanied with a report card or other proof of current school enrollment
- Department of Corrections identification card, accompanied by probation documents or discharge papers
- Social Security Card (Must be Signed)
- Car registration or title with current address
- US Selective Service Card
- Voters Registration Card
- Filed Federal tax form (with current address and signatures)
- Most recent bank statement or utility bill with current address
- Court documents with current address
The Registrar may at any time request additional documentation to support the determination of identity or eligibility of the applicant
- Must have a government issued non-expired picture ID
- If the child is 21 years of age or older, must have a notarized statement from the child authorizing issuance of birth record
The Registrar may at any time request additional documentation to support the determination of identity or eligibility of the applicant  Obtaining a Grandchild’s Birth CertificateA grandparent shall provide the grandparent’s government-issued photo ID and one of the following
- Legal guardianship documentation
- Custody or Court Order
The Registrar may at any time request additional documentation to support the determination of identity or eligibility of the applicant 
- Must have a government-issued non-expired picture ID
- Must have a notarized authorization from sibling to obtain birth certificate along with legible photocopy of the siblings non-expired government issued photo ID
- Must provide documentary evidence that siblings have at least one parent in common
- Requests must be typed on Company’s Letterhead or be included in a Court Order
- Must have Government issued non-expired picture ID
The Registrar may at any time request additional documentation to support the determination of identity or eligibility of the applicant
Obtaining a Birth Certificate by a Social Worker
- Must have a letter of authorization signed by the parent or legal guardian or a Court Order
- Social Worker must have a government issued non-expired picture IDMust present a Letter on Company Letter Head explaining the reason for the request and must be signed by a supervisor.
The Registrar may at any time request additional documentation to support the determination of identity or eligibility of the applicant
- Release papers containing inmate ID
- Department of Corrections issued ID
- Letter from the Probation Officer containing ex-inmates picture and identifying information
The Registrar may at any time request additional documentation to support the determination of identity or eligibility of the applicant
Persons that may request a death certificate include:
- Parent
- Child
- Sibling
- Informant named on death certificate
- Spouse if married at the time
- Personal representative or Executor
The Registrar may at any time request additional documentation to support the determination of identity or eligibility of the applicant
- Client must present a referral letter from the Church, Shelter or Agency that confirms his or her identity.
- Client must present a government issued non-expired photo ID
The Registrar may at any time request additional documentation to support the determination of identity or eligibility of the applicant
